update our database, reply to your questions, and improve our web site.

We do not reveal your email address or other personal information, except to complete transactions with our third party
suppliers. The ZIP code, state, and country information is used only to assess our needs in the way of our proprietary
technology. We do not sell or distribute this information to any third party. Information such as name, phone number, and
credit card numbers are collected through third party partners of nevoca.com, such as Signio, VeriSign, and their intermediaries.
Safe, Secure Online Transactions
nevoca.com is dedicated to protecting your information from unauthorized third parties. We use Secure Socket Layer (SSL)
technology to encrypt the personal and credit card information, and data you send us when using our database. SSL is a
security standard supported by Microsoft Internet Explorer (3.0 and higher), and Netscape Navigator (2.0 and higher).
Your information and data is encrypted for transit over the Internet. In addition, all information is stored in a secured
database behind a firewall. We use secure web pages to take all data entry and to display prescription details. None of
your information or data can be accessed by or released to any third party. We do utilize the services of AT&T to host our
server in order to insure the security of your information. It is important for our customers to know that AT&T does not
have access to any information that is stored on our servers.
Credit Card Fraud Protection Policy
Your credit card company can only hold you liable for up to $50.00 of fraudulent charges. We charge your credit card on a
monthly basis, subject to a monthly membership fee and transaction fees. Your credit card company will have access to combined
billing summaries only for the purposes of processing your statement.
Special Relationships
nevoca.com has a number of relationships with business partners. These business partners provide a number of different services.
Signio provides credit card transaction services for nevoca.com. In doing so, the company collects sensitive information about
users. Signio does not share information about users who make credit card purchases with
nevoca.com, it only informs nevoca.com
when a transaction has been authorized. Please read Signio’s privacy policy for more information.
Your Personal Information
nevoca.com recognizes your right
to confidentiality and is committed to protecting your privacy. When you enroll
as a member of nevoca.com, you
will create an account that includes name, mailing address, email address, credit
card number and expiration date, and Federal license number. We use the user’s
name and federal license number to manage registrations. This information is
not made available to third parties, unless we are compelled to do so by law
or in good-faith belief that such action is necessary in order to conform to
the edicts of the law or comply with a legal process served on our website.
We do not reveal your email address or other personal information, except to
complete transactions with our third party suppliers. We use your information
to verify your identity through a third party database prior to allowing you
access to nevoca.com’s services.
On rare occasions it is necessary to send out a strictly service or system related
announcement. For instance, if our service is temporarily suspended for maintenance
we might send you an email. Users may not opt-out of these communications. However,
these communications are not promotional in nature.
How to Update Your Personal Information
If you would like to review or revise the information in your account, you
may access such information by selecting “member log in” and clicking on the
“Accounts Management” button. A valid User Name and Password is required for
"member log in" and access to the "Accounts Management"
button.
Patient Information and How it is Used
Physicians and Pharmacies are able to enter specific patient information such as: full name, social security number, date of
birth and gender. Based on this input, a patient specific prescription history is generated containing medication, strength,
quantity, and dosage instructions. This information is stored solely for the benefit of the physicians and pharmacies.

does not have access to the patient information or their prescription history and does not use this information for any purposes.
The prescription history information exists solely for your benefit and to enhance our service to you. We have taken every step
to ensure that patient confidentiality is maintained. Additionally, a pre-defined patient consent form has been created for both
the physician and the pharmacy. The patient must sign this consent form before any transaction can take place.
By signing the form, the patient acknowledges their understanding of their personal information being stored on
nevoca.com’s
server.
Report Generator
The information that you entered about your patients and their prescriptions can be used to generate reports regarding their
prescription histories and the number of prescriptions that were written for a particular time frame.
nevoca.com does not have
access to the information contained within these reports and does not use this information for any purposes. The report
generator exists solely for your benefit and to enhance our service to you. We have taken every step to ensure that patient
confidentiality is maintained.
Messaging
In order to facilitate communication between doctors and pharmacies, nevoca.com offers an internal messaging service.
This service exists solely for your benefit and nevoca.com does not have access to any of the information contained within
the messages.
Cookies
nevoca.com uses cookies to keep track of who is logged onto the system. Once a user logs off the
nevoca.com system, the session
cookie will be deleted. If a user chooses not to accept cookies, he/she will not be allowed to log in as a
nevoca.com user. Cookies are not tied in to personally identifiable information.
Log Files
nevoca.com uses IP addresses to administer the site, track user’s movements and gather broad demographic information for
aggregate use. No user definable information is stored and IP addresses are not linked to any personally identifiable
information.
Receiving Email
nevoca.com will contact you by email when you enroll as a member, or to respond to any questions you may send us via email at
info@nevoca.com. If there is a problem or question and we are unable to reach you via email, we may telephone you or reach you
by fax. nevoca.com may also send you email to provide you with information on upgrades or enhancements to the
nevoca.com system.
Notification of Changes
If nevoca.com changes how we use your personally identifiable information that differs from what is stated in this privacy policy,
we will notify you by email 30 days prior to the change in order to gain your consent.
